JOB DESCRIPTION

 

 POST: Young Carers Family Support Worker

HOURS: 37.5 hours per week (to incl. evenings and weekends / flexible working pattern essential)


MAIN PURPOSE OF JOB:

  • Provide a support service to young carers and their families across Cardiff working in partnership with the Support 4 Families Team and other statutory and voluntary organisations
  • To build positive relationships with the young carers and their families to assess their needs and provide direct support to young carers families through advocating on their behalf for the undertaking of care assessments/support plans, benefit checks, form filling, listening by ear service or by any other means that supports the needs of individual young carers families
  • To provide a varied programme of support to young carers families
  • To understand the needs, rights and responsibilities and aspirations of young carers families and make positive, non-judgmental relationships with them
  • As requested, work with schools to identify and support young carers, this may include assemblies, attending school events, help to raise awareness of young carers in schools
  • Use social media and new technology to ensure young carers feel connected and develop a web- presence to reach out to young people

MAIN RESPONSIBILITIES:

  1. To deliver services to young carers families that have been identified to be in need of additional low level support, working in partnership with the Support For Families Team within Cardiff Council
  2. To develop a range of support services and programmes for families, this can include but is not limited to drop-in support, training programmes, family fun days and events and health and wellbeing sessions
  3. Work with local and national partners to support young carer’s families to gain access to appropriate information and advice in relation to their needs
  4. Work with project team to identify areas of unmet need and develop ideas
  5. To maintain an online presence through our social media platforms
  6. Liaise with a statutory and voluntary organisations to identify opportunities for partnership working
  7. Publicise and promote the young carer’s project with all relevant agencies and organisations
  8. Maintain up to date records of young people identified and supported using systems already developed by YMCA Cardiff
  9. Conduct on-going evaluations of service using feedback from the young carer’s families  and professionals
  10. Provide progress reports and regular work plans for line manager, Board of Directors and funders as required
  11. Promote equality and the valuing of diversity within the project
  12. To have a good understanding of current issues and legislation affecting young carer’s and their families
  13. Participate in regular team meetings with YMCA Cardiff Group staff and 1-2-1 supervisions with line manager, to attend Support For Families team meetings when appropriate
  14. Have a collaborative and flexible approach to work
  15. Undertake any other duties necessary to further the aims and objectives of the project
  16. To promote the values and ethos of YMCA Cardiff and Cardiff Council
  17. To carry out such duties as may be agreed between the Line Manager and the worker.
